These forms contain very important information about services being provided. The forms contained here are for your information only. Clients will receive electronic fillable versions of the appropriate forms upon scheduling an initial appointment.  Forms are listed based on the modality of therapy sought (i.e. individual, couple, or family therapy). PLEASE NOTE THAT NOT ALL FORMS APPLY IN ALL CASES
